Prednisolone pharmacologic effect. What does Prednisolone do? Action time

Prednisolone is one of the most prescribed drugs with the anti-inflammatory action. The drug inhibits a cause of the inflammatory process and does not inhibit just the symptoms unlike the non-steroid anti-inflammatory drugs.

What does Prednisolone do

The mechanism of the action is still not fully studied. We can say that it affects at all stages of the inflammatory process, inhibits the synthesis of the inflammation mediators – cytokines and histamine, and prevents the migration of the immune system cells.

This process is conditioned by the fact that Prednisolone is an analogue of the biologically active hormone of the adrenal cortex synthesized in the laboratories. These hormones have a broad therapeutic activity. The anti-inflammatory, anti-allergic, anti-shock, and immunodepressive properties of Prednisolone are especially estimated in the medicine.

Main characteristics of Prednisolone:

  • A suppression of the natural protective properties of the body to inhibit the acute inflammatory or allergic reaction reduces a formation of the antibodies, blocks the release of the interferons.
  • A redistribution of the fatty and muscular tissue to restore the functions of the body.
  • A reduction of the protein synthesis and an increase of the water retention in the body.
  • A reduction of the capillary permeability and a narrowing of the blood vessels.
  • It affects the synthesis of the prostaglandins out of the arachidonic acid, cytokines.
  • The maximal block of the histamine synthesis.

A therapeutic effect of Prednisolone develops within several hours from the moment of the use of the first pill. Therefore, the drug is considered the most effective during the suppression of the inflammatory process. In spite of this, Prednisolone is not prescribed for all indications.

Indications for use. What is Prednisolone tablets used for?

The use of Prednisolone is vast as well as the pharmacological action. However, the medication is used only when the non-hormonal drugs are not able to cope with the pathological processes in the body.

What is Prednisolone tablets used for

One of the indications where Prednisolone is an irreplaceable medication is an allergology.

Prednisolone is prescribed to reduce the symptoms:

  • Quincke’s disease;
  • Allergic shock;
  • Serum disease;
  • Severe urticaria.

Prednisolone is able to block the allergic reaction of any severity.

The second indication where Prednisolone is very important is a treatment of the inflammatory reactions:

  • Arthritis during gout and psoriasis;
  • Bursitis;
  • Neuritis of the visual nerve;
  • Inflammatory diseases of the airway tracts;
  • Polyarthritis;
  • Bechterew’s disease (Marie Strumpell disease) or Ankylosing spondylitis (AS), and others.

There are severer diseases that need an instant indication of Prednisolone:

  • Brain edema;
  • Adrenal insufficiency;
  • Kidney diseases of the autoimmune genesis;
  • Hemolytic anemia and hypoplastic anemia (Aplastic anemia);
  • Tuberculous meningitis;
  • Crohn’s disease;
  • Hepatitis;
  • Psoriasis;
  • Atopic and seborrhea dermatitis.

Prednisolone shows good results in the oncological diseases as an auxiliary product especially during the high content of the calcium in the body.

Prednisolone may be used during the transplantation of the organs as an additional drug to reduce a risk of the transplant rejection. It is taken for a long period, so that the immune system does not produce the antibodies, and the new organ is taken. Moreover, this drug shows a high efficiency both during the prevention of the transplant rejection, and to reduce the rejection during the immunity activation.

Prednisolone dosage for adults and babies. Prednisolone 5mg uses

The dosage regimen of Prednisolone is based on the prior patient’s examination. As a rule, the dose is individual according to the body mass of a patient, type of the pathological process, patient’s age, and a general clinical pattern.

Prednisolone 5mg dosage

Adults Children at the age of 3 to 12
Initial dose during acute inflammatory processes 30 mg per day 0,1 mg per 1 kg body mass
Maintaining dose during the chronic inflammatory processes 10-20 mg per day 0,5 mg per 1 kg body mass
Maximal daily dose as clinically indicated 100 mg per day 2 mg per 1 kg body mass
  • A daily dose up to 15 mg a day may be taken once. Higher doses should be divided into 2-4 equal parts.
  • Doses more than 40 mg per day should be divided into 4 equal parts in the dose of 10 mg Prednisolone and taken with the same interval – every 6 hours.
  • A one-time dose should not be more than 15 mg for better digestion in the gastro-intestinal tract.

The dose should be higher during the acute pathological states compared to the maintaining therapy. As soon as the intensive therapeutic effect is achieved, and the symptoms are improved, the daily dose is gradually reduced by 2,5 mg or 5 mg.

The dose is reduced once per week, in rare cases, once per 4-5 days. In case of the sudden decrease of the daily dose, a risk of the disease symptoms aggravation grows. In this case, the dosing regimen has the initial level, and the second decrease of the daily dose is started in 1-2 weeks.

Prednisolone contraindications

Admittedly, any hormonal product has a big list of the contraindications. There are conditional restrictions recommended for the observance, and there are strict contraindications.

Of course, the conditional contraindications take a back seat during the severe allergic shock, and the main task is to save a human. However, do not neglect the contraindications, especially during the self-treatment with Prednisolone.

The use of Prednisolone is prohibited during:

  • Allergic reaction to Prednisolone;
  • Intolerance of lactose or fructose that is in the pills and may cause an allergic reaction;
  • Pediatric use under 3;
  • Pregnancy.

The conditional restrictions include:

  • Diseases of the gastro-intestinal tract: ulcer of the stomach, gastritis, and esophagitis;
  • Infectious and fungal infections: herpes zoster, chickenpox, tuberculosis, amebiasis, and trichomonas;
  • Severe types of the endocrine disorders: diabetes mellitus, thyrotoxicosis, and hypothyroidism;
  • Severe chronic renal and/or hepatic failure;
  • Diseases of the cardiovascular system in a severe type: arterial hypertension, chronic heart failure.

These restrictions are conditional only when there is a threat to the life because of the rapid development of the pathological process. In the rest cases, a patient should read about the list of the contraindications, and consult a doctor, if any similar states are detected.

The use of Prednisolone in pregnancy and lactation

Prednisolone is not prescribed during pregnancy, if possible. Due to the changed metabolic processes, a disorder may happen in the development of a baby. If it is impossible to refuse from Prednisolone during pregnancy, it is taken in the minimal dose, and only during 1 and 2 terms.

Prednisolone in pregnancy

The use of Prednisolone is cancelled in the 3rd term of the pregnancy because at this stage, there is a high risk of the insufficient development of the fetus. It is a severe pathological state that needs a replacement therapy from the first days of a baby’s life.

Admittedly, Prednisolone penetrates into the breast-milk and reaches the baby during the breast-feeding. The use of this drug is not recommended during the growth and development of a child at the age of under 3 in order to minimize the side effects and changes in the growth.

Therefore, if a woman has to take Prednisolone during the lactation due to the medical reasons, stop breastfeeding.

Prednisolone for kids. Is prednisolone safe for babies?

Prednisolone use is allowed children at the age of over 3. The use of the drug at early age is not recommended because the drug can cause the metabolic and endocrine disorders during the active development of a baby. Prednisolone is allowed in the minimal dose 0,5-1 mg per 1 kg body mass at the age of over 3.

Taking Prednisolone actively excretes the calcium from the body, and it may slow the growth and a development of the bone tissue of a child. Due to this reason, the additional additives with calcium must be taken during the treatment.

Children should take Prednisolone for a short period. A risk of the hormone changes is low during the short medical courses. In spite of this, a child is regularly taken the blood tests and the hormones are controlled. If any abnormalities, the further treatment should be reconsidered.
